The cost of primary health care services for New Zealand Citizens is subsidized to varying degrees by the government. In order to access these subsidies and be seen at our practice we encourage you to enroll with Dee Street Medical. Please note that you can only enroll at one practice at a time.

Allow 10mins to complete your online enrolment.
Please have on hand:
NZ Citizen: Passport OR Birth Certificate and Drivers Licence
Non NZ Citizen: Passport and Visa
Once we have received your enrolment we will email you our New Patient Health Questionnaire. Please complete this and return to us via email.
As part of the enrolment process we need to obtain your medical history. Once we have received your notes we will inform you via text that you can book your *New Patient Appointment
Please Note: this process can take 7 - 10 business. Make sure you have made arrangements for this, including having enough of your regular medications before starting the process.
*This is an extended length appointment which includes a 10 minute appointment with a health assistant, followed by 20 minutes with your new GP. This appointment helps us capture your key healthcare information and get to know you.
Manage My Health
When you enrol, our reception team will also register you for Manage My health, a secure online patient portal.
Through the portal you can book appointments, view results, order repeat prescriptions and more.
You will need an individual email address (not a family email address) and photo ID, which our team will use to register you. You'll then receive an email inviting you to activate your account.
